Why do interviews not always get the best candidate?

Interviews are quite an artificial situation.
Many candidates do not perform well in them and show off their skills.
Doing the job is very different to an interview and many can't say how good they are in interviews whilst those who are not good at the job are very good at presenting themselves well due to either confidence or experience or a mix of both.
Also there is the case of poor interviewers who are unable to identify the best candidates.
