All exercise has a risk of injury, but there are two very common causes.

The first is not performing the exercise correctly - or not having warmed up before hand and not warming down after wards.

If you ever have any doubt about how to perform a particular exercise you should always research it first and if necessary ask an expert such as a gym instructor to ensure that you get the right technique.

The second major cause is that people concentrate on certain muscles in their body, and hence others become weak in comparison and cannot cope with the strain that the other more developed areas can.

The solution here is that you should concentrate on getting a balance, and not foresake the weaker muscles in your body. Try to train in a balanced manner so that all areas of your body develop in proportion.
