What does it mean when you get constant sharp pains in the heart and also constant dizzy spells?

I am 13 and keep getting sharp pains in the heart and dizzy spells they happen at different times the dizzy spells happen when i stand up and the heart pains will happen at anytime i have had the dizzy spells since i was 12 and the sharp pains a few weeks before i turned 13 please help.
