If you have several addresses, for instance if you are a university student, then you need to give the dates when you are at each address. Imagine how you would feel if you were called to interview, but were completely oblivious to that fact since you were at home, when the letter was sent to your university address! If you have an email address, include it, and also give a phone number for every address that you give, if possible. That way they are sure to be able to reach you if they require that you attend interview, which hopefully they will!
