What is the structure that regulates what enters and leaves the call?

Presumably you mean what leaves the cell rather than what leaves the call?

Well assuming that you do, the answer to this is that the cell wall is what decides as it is what is outside the cell, and this is a semi permeable membrane that can control what comes into the cell and block what it wants but let out what it wants too.
